Anaphylaxis Emergency Plan

If your child is anaphylactic please:

​

• Print anaphylaxis plan from the button below

• 2 Epinepherine Auto-Injectors must be provided - One the child can carry and an emergency one to be kept at the child care centre

• All Epinephrine Auto-Injectors must have a prescription label from the pharmacy on them

• Dr. Stamp must be on first page of plan with the Dr. information

• Parents must review plan and train use of Epinephrine Auto-Injectors to staff, students and volunteers on duty prior to child starting

• Parent's must authorize the Centre Supervisor and designate to train any staff, student and volunteers who were not at the initial parent training
